You can contact your insurance carrier directly to submit a claim for the eligible supplies purchased.

You can do this by obtaining the contact information for your carrier on the back of your insurance card. Once you locate the number for customer service. Prepare for the call by gathering the following information that is located on your receipt:

  • Your insurance card
  • the CPT code for the each item you want to put on the claim.
  • The cost of the item purchased
